CEU REQUEST FORM

The following form should be used in requesting continuing education units (CEUs) for Diocesan-sponsored workshops. Upon receipt of the form, the contact person will be given authorization to conduct the workshop described. Upon completion of the workshop, names, addresses, and social security numbers of participants can be submitted using Form 2.

Participants should be reminded that the current cost of CEUs is $10.00. Checks are made payable to: "Office of Catholic Schools".

Application for CEU program approval should be made at least two weeks prior to the program. Participants names must be submitted 10 days after the approved program has taken place. Letters of participation will be issued upon receipt of CEU payment.
